This form is a required part of the non-laboratory departmental Exposure Control Plan.

The OSHA Occupational Exposure to Bloodborne Pathogens Standard requires that department heads prepare an exposure determination. This should be made without regard to the use of personal protective equipment. The exposure determination shall contain

  1. a list of job classifications in which all employees in those job classifications have occupational exposure
  2. a list of job classifications in which some employees have occupational exposure
  3. a list of all tasks and procedures or groups of closely related tasks and procedures that are performed by employees in job classifications listed in category b, above, in which occupational exposure occurs
