Internal Routing Form for Industry Sponsored Clinical Trials (OSR-CT)

A streamlined version of the traditional OSR-1 proposal routing form, geared specifically to industry sponsored clinical trials. An electronic version of the OSR-CT form and instructions for its use are available on the OSR Web site; paper copies may be obtained from the Chicago or Evanston OSR offices. The OSR-CT incorporates several unique features, such as a section for pre-spending requests, and it eliminates traditional OSR-1 form data fields that are not applicable to this type of sponsored activity.
