Submit the following documents to OSR at least five business days before an NIH deadline:

1. PHS 398 forms: modified to incorporate the new modular format.
2. OSR-1 form: completed and signed.
3. OSR-100 form: completed and signed for each investigator
4. On-campus Modular Budget Tabulator form
   or
   Off-campus Modular Budget Tabulator form

If there are subcontracts involved, submit the following for each subcontract:

1. Detailed budget for Year 1 (spreadsheet acceptable)
2. Multi-year budget, by major category (spreadsheet acceptable)
3. Statement of Work (to be done by subcontractor)
4. Consortium letter (indicating approval of the proposal by the subcontractor's OSR office and their willingness to enter into a consortium arrangement if the project is funded)
