Required Documents

For all research proposals:

• Complete, endorsed, Proposal Submission Form 
  • OSR-CT for industry-sponsored clinical activities
  • OSR-1 for all other activities
• Original proposal and copies
• Proposed Internal Budget for a Sponsored Project (OSR-3)

Also required for proposals to NIH or NSF

• Significant Financial Interest Disclosure (OSR-100)

Also required for proposals involving human subjects or animals

• IRB &gt ACUC forms

Also required for researchers with joint Northwestern/VA appointmants

• Memorandum of understanding

Also required to issue or modify a subagreement

• OSR-6 or OSR-7

Individual funding programs may require additional material to be submitted.  Contact your OSR Research Administrator to verify program requirements.
