PROTOCOL REQUIREMENTS for PERSONNEL APPROVAL

IACUC Training Services and Occupational Health Safety Program (OHSP)

All project investigators and research staff handling and caring for animals are required to take the basic IACUC
on-line training and be enrolled in the Occupational Health and Safety Program (OHSP). In addition,
anyone seeking facility access CCM or having contact with animals must be listed on an approved protocol.

Training Services

All Principal Investigators (PIs) and research personnel must complete the basic on-line training provided
through the AALAS Learning Library.

  • Contact Bruce Roberts at OPRS/IACUC at 312-503-9339 or 312-503-2618 or by e-mail at

b-roberts@northwestern.edu. Provide the spelling of your name and he will assign a Username and

Password. You will then have a Northwestern University account with AALAS Learning Library, which

you will need in order to take classes.

  • Log onto the site at www.aalaslearninglibrary.org, click on “Subscriber Center” or “Personal Access”
    in the upper right corner and update your personal information contained in “My Profile”. It is important
    to enter your Northwestern University NetID. This is essential for the transfer of you transcripts to our
    record system. You can update your profile at any time.

  • To take classes, click on “Instutional Animal Care and Use Courses” this page contains tracks of courses

for Northwestern University animal users. The tracks contains required courses for each species as well as

recommended courses.

 

All training modules must be completed by all individuals listed on an Animal Study Protocol or an
Addendum prior to IACUC submission.


Occupational Health Safety Program (OHSP)
All individuals listed on an Animal Study Protocol must initiate the OHSP process prior to IACUC submission.

Occupational Health clearance/enrollment can be completed concurrently with the IACUC review approval

process. However, the official approval cannot be released unless the enrollment requirements are met.

To enroll in OHSP please complete and submit the required medical questionnaire and enrollment request form.

Upon completing the forms please call the chosen service provider for processing the OHSP enrollment.

*PI's and research staff must undergo the medical questionnaire review for enrollment. If working with

nonhuman primates, an initial TB test is also required. Further services may be determined to be necessary
by OHSP to obtain clearance/enrollment.


Submission procedures for Occupational Health

Corporate Health - Chicago campus
The medical questionnaire should be completed and submitted by fax or personal delivery to Corporate Health
for review and clearance. Corporate Health, Suite 9-240, Galter Pavilion, 201 East Huron, Chicago, IL 60611,
Ph: 312-926-8282, Fax: 312-926-1787.

To avoid direct patient billing, complete and submit the enrollment authorization form to OPRS/IACUC via

fax to 312-503-0112 or drop it off in the OPRS/IACUC office (Rubloffl, 7th floor, Chicago, IL 60611).

OMEGA - Evanston campus
Complete the medical questionnaire and health history form then submit to OMEGA. OMEGA, Suite 800, 1000

Central Ave, Evanston, IL 60208, Ph: 847-657-1700, Fax: 847-657-1799.

To avoid direct patient billing, complete and submit the enrollment authorization form to OPRS/IACUC via

fax to 312-503-0112 or drop it off in the OPRS/IACUC office (Rubloffl, 7th floor, Chicago, IL 60611).

General Information

  • If you are a new PI, prior to working with animals, it is required that a Animal Study Protocol (ASP) be submitted.
  • If you are adding personnel to an already approved protocol, you will need to submit an Addenda form/checklist.
  • For additional training needs or for inquiries regarding obtaining access to the vivarium please contact
    the Center for Comparative Medicine (CCM).
